-1 passport photograph with the head measuring between 1 inch to 1 3/8 (not usual dimension, they have specific dimension)
-Passport with 6 months validity
-Bank statement of last 6 months
-Males applicants between the ages of 16 45 must complete form DS157
-Passenger to present personally for finger printing
-Fees not refundable
-Payment for visa fees: should be made at the MCB Bank Counter, you will have a receipt to be presented at the USA Embassy
